Dad has a 04, his would only work on setting 5, such as yours.
I have a 05, mine would work on 1-4, but not 5, then not 4..
New resistors fixed them both.
Mine was done when the truck was nearly new (covered by the tail end of my warranty) Dad did his around Christmas time. I'm sorry I can't enlighten you on the exact location..
I can say that both the 2 and 4 hole only use 2 mounting screws.. The 4 hole has another "bracket" that the resistor hooks to, then the bracket screws in. We found out because dad ordered a 2-hole (didn't look online and compare pictures) and it wouldn't work.. "But there was only 2 screws.." Which is when NAPA parts man said they both used 2 screws.
I would however, as instructed, check out the connection going into the resistor and make sure you are getting power to the resistor.. We changed one in a Saturn (because dad says "resistor was bad in the pickup, so this heater that don't work has a bad resistor, right??" NO!!) and it fixed nothing.. Probed the harness and found that the resistor only got power on one setting of the switch.. New switch, back in business.
As I say.. some people just throw parts at stuff till it's fixed.. others analyze the problem and fix it cheaper and faster.. Dad prefers to throw parts.. I prefer to find the issue, then toss the parts at it
