Heat or lack of oil thereof usually causes spun bearing, Without oil and cooling and lube the parts grow till they grab each other. Since the insert is not that tightly held ,it spins. Of course overload/overrev or a loose rod bolt/nut could do same . Lastly I had a Ford van that did it,No overload no over rev,plenty of oil and oil pressure I guess it just got tired or perhaps factory assy error.
