Harmonic balancer Install

Ok guys, time to swallow the pride and fess up. I made a mistake. Not sure how or why, but I did and now I need some help. I installed the crank key and pulled the balancer on with the crank bolt (like you are not supposed to do). When ran, the crank pulley wobbled. I retorqued the bolt and it had no effect. I then pulled the front end off and found the crank bolt only hand tight. The balancer also slid off easily. This is what I found.
I need to know what I did wrong that allowed the keyway to rotate as the balancer was installed and break the rear lip off of the balancer. It was a $100 dollar mistake and I cannot afford to make it again. Any ideas? Thanks in Advance, Aaron
