Based on your description it's a problem with the countershaft - either drive gear (or it's mating gear on the input shaft), or else the bearings supporting the countershaft. Reason is the countshaft carries power in gears 1 thru 4. But it carries no load in 5th, nor when free-wheeling. So worn parts cause no noise in 5th and while coasting. Unlikely it's a bent fork problem. Unfortunately I think you're gonna have to go back inside the tranny and take a close look at those countershaft drive gears and bearings - sorry!!
